This referral form is for use by Probation staff only. Referrals submitted from outside of Probation cannot be accepted.

This form is used to refer individuals into the Bridge-way Navigator service. Bridge-way is a highly targeted intervention for people returning to Bradford from custody who have been sentenced (not on remand) and are at high risk of recall, particularly where this is linked to accommodation instability and multiple unmet needs.

This is an intensive, relational service delivered by a small specialist team of two Navigators, each working with a limited caseload of approximately 10 individuals. It is therefore essential that referrals align with the eligibility criteria and are focused on those most likely to benefit from this level of support.

The eligibility criteria are embedded within this form and must be met in full. A number of fields are mandatory to ensure that referrals can be reviewed efficiently and decisions made at pace. Referrals cannot be submitted unless all required fields are completed.

Once submitted, the referral will be received directly by the Bridge-way team, who will review the information and make contact with the referrer to confirm next steps.
